  Print and complete it carefully.
  
• Prepare your donation cheque at the following order :

  and write on backside :
  "Donation to support
  the Clarisses'establishment at Ronchamp". 
  
• Insert cheque and donation form inside an envelope
  and send it through the Post Office,
  to the following address :

• A receipt for tax refund will be sent directly to you
